Once in Cultivate U, click the List Icon in the left menu panel to access your Timeline.



 Click GO next to the “Complete Evaluation & Hold Discussion” Task to access the form.

 

The form is separated into tabbed sections and the action bar, which includes options to Save, Submit, Print/Create PDF, and access any Notes you made throughout the year. 



PERFORMANCE GOALS

Once the form opens, click on the Performance Goals section to review each of the performance goals that the employee entered for the year. Click the Expand All button to view the details of each goal.


If your employee has not yet updated the Completion Status for their performance goals, you will need to change the Completion Status (and date, if needed) to describe the current status of the goal at the time the evaluation is being completed (i.e. on track, completed, behind or cancelled). The Impact section can be updated, as needed, to reflect how the performance goal impacted the department, unit, or overall College. Additional supporting information should be documented in the Comments field.


PERFORMANCE EVALUATION COMPETENCIES

The next section includes all of the competencies that managers are to be evaluated on. The instructions include information on the rating scale options and when each should be used.
*If you would like the employee to submit their thoughts regarding the competencies prior to the evaluation discussion, you can have them fill out the Evaluation Preparation Form and submit to you.

Click on the Expand All button, or the arrow next to a specific competency to view the performance indicators and mandatory comments box for each competency. 

Achievement of competencies is verified through observation of employee activities and observation of and interaction with the employee over a period of time (cumulative). Supervisors are required to select a rating for each competency as well as provide objective examples in the Comments box to support each rating.



OVERALL COMMENTS/RATING

This section includes the overall comments from the supervisor as well as the overall performance rating. The instructions include information about when each of the different ratings should be used as well as a link to the Performance Improvement Plan, if needed. (Note: If a Performance Improvement Plan is completed, please attach the signed plan to the evaluation using the Attachments section.

 

Enter your Overall Comments as well as the Overall Rating based on the instructions provided.


EVALUATION DISCUSSION

Once all of the sections of the evaluation have been completed, you are ready to hold the evaluation discussion with the employee to review the information. If you would like the manager to review the form and information before your meeting, you can click the Print Preview button to either Print or Export to PDF and send to the manager. Otherwise you can bring up the form on a computer or print a copy to bring along to the meeting.

  

NOTE REGARDING DEVELOPMENT PLANS 

Although the employee’s development plan activities (professional growth goals) are no longer a part of the annual evaluation form, please take time during the evaluation discussion to review the current year’s plan and start the discussion around next year’s plan. Each manager should then enter his/her development activities in the Development Plan for the upcoming year and submit the plan for approval by mid-May.
 

 

SUBMIT FORM FOR EMPLOYEE’S SIGNATURE

Once the evaluation is finalized and you are ready to send the form to the employee for his/her signature and comments (optional), click the Submit button in the action bar then click Yes in the confirmation window. *NOTE: The supervisor signature will be added as the last step after the employee has signed and submitted the form.  


EMPLOYEE SIGNATURE

The management employee will now see the evaluation step in their Timeline stating that they need to sign their evaluation. When they access the evaluation, everything will be in read-only mode for them to review. They will then need to go to the Signatures tab to sign and acknowledge the discussion. They also have the option to add additional comments, if desired. Once they have signed the form, they will need to click the Submit button to move it to the final step.


SUPERVISOR SIGNATURE

After the employee has signed and submitted the evaluation form, you will see the “Sign and Submit Evaluation” task in your Timeline.

 

Once in the form, click on the Signatures tab, view any added employee comments and then click the SIGN button to fill in your electronic signature and date. You will also need to fill in the date on which you met with the employee to discuss the evaluation.


 

Finally, click Submit to finish the evaluation process.


APPROVING PERFORMANCE GOALS FOR THE UPCOMING YEAR

Once a manager has submitted their goals for the upcoming year, you will see a new task on your home page to Approve Performance Goals for the employee. When you open this form, you will be able to review the performance goals entered by the employee and add or edit any of the goals as needed. After you have approved the goals by signing the form, click Submit so complete the step.
